*We do our very best to keep our patients informed of schedule changes and office closures, however, sometimes we have wrong numbers, are unable to reach each patient when changes occur, etc. Therefore, please try to contact the office in which you plan to visit, before you make the trip, to confirm we are open and operating under normal business conditions*

You may enter as a stranger but you will quickly become part of our large growing family!
Some insurance companies require a referral from your primary care provider (PCP) before they will pay for services. Please be sure to provide us with the referral or the name of your PCP so we may assist you in obtaining the referral. Most PCPs require you to have been seen in their office within the last 3-6 months before they will agree to provide a referral.
Along with the referral you will need to bring ALL of your most recent insurance cards and your current medication record.
If you are unable to provide us with your insurance cards or you do not have insurance payment will be required at the time of service.
